About Critical Time Intervention (CTI)
Created over twenty years ago, the original Critical Time Intervention (CTI) model was a time-limited intensive case management approach designed to reduce the risk of recurrent homelessness among single adults transitioning from shelters to housing. In a series of three timed phases, CTI aimed to connect these vulnerable individuals to crucial services and supports and assist them in navigating complex systems of care during the transition period.
Over time, CTI has been used to help a range of vulnerable populations mobilize supports during care transitions. It's goal remains to create deep, lasting connections to supports that will endure well beyond the end of the active intervention period. In an implementation pilot conducted at VA Greater Los Angeles, we adapted CTI for the VA Grant and Per Diem case management aftercare grantee program.
