The VA Grant Per Diem Case Management “Aftercare” Program is operated by VA’s community partners to provide 6 months of case management for homeless-experienced Veterans undergoing housing transitions; implementation of CTI in Aftercare can standardize and improve case management services in this program. How does CTI benefit Veterans?
CTI is delivered in three time-limited phases of decreasing intensity. Services are provided by a single case manager (“CTI Specialist”) who delivers field-based care that helps Veterans mobilize resources and support. Participating Sites
Housing Transitions QUERI is implementing and evaluating Critical Time Intervention (CTI) in 32 Grant Per Diem (GPD) case management aftercare grantee sites in 7 VISNs across the nation over 4 years. Implementation at the 32 sites will be completed in three separate waves, each wave lasting 9 months.

Wave 1
In Wave 1, we implemented CTI in 11 GPD case management aftercare grantee sites (blue stars on the map), which are affiliated with 8 VA facilities (listed below) and 6 Veterans Integrated Service Networks (VISNs). VISN 1

In Wave 2, we implemented CTI in 11 GPD case management aftercare grantee sites (teal stars on the map), which are affiliated with 10 VA facilities (listed below) and 6 Veterans Integrated Service Networks (VISNs). VISN 1
